Collection Site Only (DOT and NON-DOT)
UHS Occupational Medicine may act as the collection site only for drug testing. The Company will be responsible for providing the UHS Occupational Medicine collector with Chain of Custody forms along with all supplies and equipment necessary to carry out the collection procedures including specimen collection kits, mailers/air bills and/or any other costs associated with the transport of specimens.
