Social Determinants of Health and Equity Initiatives
Health equity can be achieved when no one is disadvantaged due to a social position or other socially-determined circumstance.
Equity initiatives at UHS consist of a variety of programs based on certain individuals who experience specific disparities.
Social Determinants of Health are the conditions in the environment where people are born, work, live, play, worship, and age that affect a wide range of health, functioning, and quality of life outcomes and risks.
These factors include (but are not limited to):
- Economic Stability
- Education Level
- Social Connections
- Food Security
- Stress
- Transportation Needs
- Housing Stability
- Resources Needed
UHS strives to provide support to our patients for all of clinical and social health needs. This is accomplished by referring patients to community resources, internal programs and services provided by our own organization.
