Chenango Memorial Employee Health Office
The Employee Health Office is located in the Human Resources location on the basement floor of the hospital. The employee health nurse is Heather Esposito, RN.
Hours are from Noon to 4 p.m. on Mondays and Thursdays, 11:00 a.m.- 3:00 p.m. on Tuesdays and 7:00 a.m. to 11:00 a.m. on Wednesdays and Fridays. Appointments are not needed but recommended for your convenience.

The office provides annual health assessments and immunizations. For other health needs, you will be referred to a provider or Occupational Med.
Delaware Valley Employee Health Office
The Employee Health Office is in the hospital, in the quality office (across from HR), the IP/EH Coordinator is Jennifer Barnes.
Hours are from 7 a.m. to 3:30 p.m., Mondays through Fridays.

The office provides annual health assessments, fit test and immunizations. For other health needs, you will be referred to UHS Primary Care Walton.
Frequently Asked Questions
Why do I need an Annual Health Assessment (AHA)?
Annual Health Assessments are required annually by NYS Department of Health for all health care personal. Assessments are for the purpose of determining that you are free from health impairments which could pose a risk to you, patients or co-workers and should be completed during the employee’s month of birth.
The first annual health assessment is completed at the time of onboarding and is called a "pre-employment physical." After that, it is recommended that annual assessments are to be completed during the employee’s month of birth (EHO loves to celebrate birthdays!). No more than 12 months should elapse between annual health assessments.
What is included in my Annual Health Assessment (AHA)?
The AHA is review of your health status and the following will be reviewed and completed during your visit:
- Questionnaire related to TB screening
- Height, weight, blood pressure and temperature will be taken
- A review of your immunization history
- Vaccines and/or blood work will be offered, given or completed
- If you have direct patient contact you will need to complete a Respirator Questionnaire and N95 Fit Test will be completed
- Information on the Employee Assistance Program (EAP) will be provided
- Information on what to do if you were injured at work (Workers Compensation PPO Program)
- We can assist with a referral to a Primary Care Provider if you are in need of a PCP
Who requires a N95 Respirator FIT testing?
Per OSHA, FIT testing is to be completed annually for ALL health care workers providing direct patient care. This will be done during your Annual Health Assessment (month of birth). If an employee has a job change and now requires a N95 FIT test, please contact or use the Schedule an Appointment link for either Binghamton or Wilson Employee Health Office to schedule your FIT test.
Am I required to receive the COVID vaccine?
UHS personnel must meet one of the following criteria:
- Be (minimally) fully vaccinated for COVID-19 OR
- Have a COVID -19 vaccination declination form on file with the Employee Health Office

How can I provide proof of vaccination to Employee Health?
Proof can be dropped off, emailed to Employee.Health@nyuhs.org or faxed to Binghamton (607-296-2239) or Wilson Employee Health Office (607-763-5131).
When should I call the Medical Command Center?
Medical Command Center should be contacted directly at 607-763-5991 if you are experiencing any of the following:
- Symptoms related to COVID-19
- High risk exposure
- Tested positive with a home test
What do I do if I have had an accident or injury at work?
Any injury, accident, blood or bodily fluid exposure, needle stick post exposure IMMEDIATLEY notify your Manager/Supervisor and complete and Employee Injury Report (regardless if you want to seek treatment or not). Complete form and provider it to your manager/Supervisor.
Managers/Supervisors are responsible to review, sign report and send completed form to the Workers Compensation PPO at UHS.WC-PPO@nyuhs.org.
If seeking treatment for your injury please call 1-800-259-1753 to speak with a Workers Comp PPO Nurse who are available 24/7. During this call you will be advised on where you should go to seek treatment.
How do I schedule an appointment?

Can I just walk into an Employee Health Office?
Yes, but scheduled appointments will be given first priority. Check out our Schedule an Appointment links.
What if I just saw my Primary Care Provider for an assessment?
Annual Health Assessments with your provider are different than your EHO Annual Health Assessment. EHO Annual Health Assessments are a NYS Department of Health requirement for ALL healthcare workers. EHO AHA’s are not billed to your insurance and information entered into your Employee Health chart, including vaccinations are not shared in EPIC.
What do I do if I have been out of work for 5 consecutive days and now ready to return?
After 5 consecutive days of bring out due to illness or injury, before returning to work employees MUST report to an Employee Health Office on the day you are scheduled to work prior to your shift. Employees will need to have a return to work note from a provider with specific instructions for restrictions or limitations (if any). The EHO nurse will perform a return to work assessment and/or complete an AHA if one is due at that time. If restrictions or limitations are noted the EHO nurse will contact the employees Manager/Supervisor to review and confirm they are able to accommodate the return to work restrictions/limitations. Maternity/Paternity leave does not require a return to work note.
Return to work Drug Testing is no longer required at UHS.
How can I obtain a record of my immunizations and FIT test?
Employees will need to complete and sign a Release of Information and provide a copy of their photo ID. The completed form and Photo ID can be submitted to Employee.Health@nyuhs.org. Records can be mailed, faxed, emailed to your UHS email, or picked up in person at any Employee Health Office. Please note that records cannot be emailed outside of the UHS organization.
