The pre-participation physical is an essential component of the care provided to athletes. New York State requires a new physical exam every 12 months for sports participation.
It can be a challenge maintaining compliance with state requirements for annual physicals. Given the size and integration of UHS Sports Medicine, we are able to offer our schools a comprehensive “Physicals Day” bringing all of our Providers, Athletic Trainers, Nurses, Performance Coaches and Administrators in to your school. Our team will complete superior quality pre-participation physicals for all athletes in a fraction of the time.
You can download the pre-participation health history form here to fill it out prior to your physical.
Athletic Training Partnerships
On the Sideline
Our Athletic Trainers are contracted with local high schools, colleges, professional teams, as well as community facilities. Our athletic trainer administers on-site evaluation, injury prevention programming and management and treatment of acute, subacute and chronic injuries and/or conditions in an effort to restore function. They are ready to evaluate and are usually the first to triage an injury, being highly trained to make the right diagnosis and decisions quickly. Our athletic trainers are heavily relied on for everything from injury and illness prevention and assessment to emergency care, first aid, and concussion testing to return-to-play decisions. Our athletic trainers coordinate and communicate with team physician’s, athlete’s primary care provider, physician specialists and the coaching staff. Our athletic trainers coordinate sports physicals for teams, schools, and groups where we have established affiliations.
